According to Powell, OH, cosmetic dentist Neal Patel, DDS, even the healthiest patients can experience teeth that slightly shift and drift. "Crooked teeth make you look older because we generally associate straight teeth with a youthful, healthy mouth," says New York cosmetic dentist Irwin Smigel, DDS.</p>
<p><strong>Why teeth shift</strong><br />
Our teeth shift with age due to a lack of bone strength. “The teeth aren’t as strong as when they were younger,” says Dr. Smigel. Even if you had braces as a teenager, conditions can develop that affect the gums, forcing shifting to occur. If you are genetically blessed with naturally straight teeth and you never needed orthodontics, it’s still possible for your teeth to shift. When the bone that holds teeth in place is lost or spaces arise from missing teeth, the teeth move into the empty spaces. “The bone can also shrink, causing teeth to become loose and drift,” says Dr. Smigel. Add in years of wearing fillings and crowns, coupled with the repeated movements made from chewing and clamping down on the teeth, and shifting is inevitable. “Another cause for shifting is gum disease,” says Dr. Patel. “While it’s not usually painful, it can force teeth to become crooked and rotated because the gum pulls away from the tooth.”</p>
<p><strong>How to prevent it</strong><br />
If you have retainers, wear them. “Even if you never had braces or have not had them for a long time, ask your dentist to fit you for a retainer to keep your teeth from shifting,” says Mt. Pleasant, SC, cosmetic dentist Nicole Dahlkemper, DMD. Regular cleanings are the most effective way to prevent bone loss. Missing teeth, which age the smile tenfold, also cause your other teeth to shift, so it’s important to restore them. “If a tooth is extracted it must be replaced with an implant or bridge because nature abhors a vacuum and even healthy teeth will move to fill in the space,” says Dr. Smigel.</p>
<p><strong>What to fix first</strong><br />
If gum disease caused shifting, the issue needs to be corrected before any cosmetic work is done. “The gums will continue to cause problems if the underlying issues aren’t corrected,” says Dr. Patel. Then, once the gums are under control, the teeth can be straightened with braces or Invisalign.</p>
<p><strong>Align your smile</strong><br />
For teeth that have shifted minimally, removable aligners like Invisalign can be used. “Invisalign is one of the best ways to straighten teeth. It returns teeth to the right positions through slow, controlled movements,” says San Francisco cosmetic dentist Niloufer G. Hamsayeh, DDS. Adult orthodontics not only create a better bite, but they also lead to better functioning teeth. “Crooked teeth invite gum inflammation and recession,” says Beverly Hills, CA, cosmetic dentist Laurence R. Rifkin, DDS. “Straightening them makes them easier to clean and reduces inflammation.” Advanced shifting may necessitate conventional braces.</p>

<p>And don’t forget to clean between those teeth once a day. “Your children should be flossing between any two teeth that touch,” Dr. Hayes says. “However, many kids don’t have motor skills to floss until they are over 10 years old.” If your child needs help, try different types of interdental cleaners or put your hands over theirs to guide them and get the job done at the same time. </p>
<h3>Say no to sugary drinks and snacks </h3>
<p>As the temperature rises, it’s common for families to sip and snack during sports tournaments, festivals or nearly any community event. “Watch your family’s intake of lemonade, juice and soda,” says Dr. Hayes. “Consider sugary drinks treats to enjoy once in a while, and not often.” Instead, offer water (even better if it has fluoride) to beat the heat, or milk to drink with meals. And, don’t let summertime grazing damage your child’s smile. “Taking a break from snacking is healthy for your teeth,” says Dr. Hayes. “It allows time for saliva to bathe the teeth, wash away leftover food and get stronger.” </p>
<p>If you find yourself spending more time at home, snack smarter, and let your children tell you when they’re hungry instead of offering snacks throughout the day. “They’re not afraid to let you know when they want something to eat!” she says. </p>

I take pride in my pearly whites, brushing them two or three times daily, flossing nightly, yadda yadda. Rappaport has seen a lot of smiles, but I was sure he’d be impressed.</p>
<p>Wrong.</p>
<p>“Your gums could be healthier,” he tells me afte <span class="show-more hide">r the routine flossing. Apparently I don’t floss firmly enough, evidenced by the blood I had to spit into that cute little sink beside my chair. “The real way to have healthy teeth is to ensure you have healthy gums,” doc continued. “It’s where the teeth are lodged, after all. Unhealthy gums can lead to tooth loss.” I’m thinking “D’what?!”, like the kid who crammed all weekend for the final exam and somehow still only got a B.</span></p>
<p><span class="show-more hide">Rappaport reassured me, though, that I’m well on my way to a perfect oral grade. I’m no candidate for tooth loss, but I needed to brush up on my gum care. I left wondering how I could do better next time, but also more curious about the gums in general. I hadn’t heard any dentist emphasize them as much before, aside from general flossing rules, so I reached out to Dr. Rappaport with a few questions.</span></p>

<p><strong>1. Why is gum health just as important (if not more important) than tooth health?</strong></p>

<p>“Our gums are the foundation to healthy teeth and a beautiful smile. When gum tissue becomes infected with bacteria from dental plaque and tartar, bone loss and tooth loss occur. This advanced gingivitis is called periodontal disease. Periodontal disease also puts patients at a higher risk for other medical conditions such as heart disease, dementia, and diabetes. And in [pregnant] women, it can also lead to premature birth.”</p>

<p><strong>2. What are the signs of unhealthy gums? How do we know if they need more attention?</strong></p>

<p>“Do your gums bleed when you brush your teeth? Inflamed and bleeding gums are early symptoms of gingivitis and periodontitis. Healthy gums are firm and pink.”</p>

<p>“Smoking and tobacco use are the big offenders. These patients are at a higher risk for gum disease. Also, poor nutrition and stress will compromise gums. These habits are putting you at a higher risk for gum disease. Plaque (that sticky stuff on your teeth) begins to form soon after brushing, and if it isn't removed, it begins to calcify and spread below the gum line. The bacteria from plaque and tartar causes the gums to swell and bleed as our immune systems kick in to try and fight it.</p>

<p>"Also, don't skip flossing because it ‘wakes you up’ before bed, or only brush for a few seconds when you’re rushing out the door. And ignore the anti-flossing rumors."</p>

<p>“Good oral hygiene is your first and best defense against gum disease. Brushing twice a day, routine flossing, and adding a rinse helps keep plaque and bacteria from sticking around. Flossing below the gum line is important to remove food debris and plaque where our toothbrushes can't reach.”</p>

<p>“Make sure you are keeping up with regular visits to your hygienist and dentist, too. Most people should go every six months. Patients with crowded teeth or hard-to-reach areas, active orthodontic patients, smokers, and tobacco users are at higher risk for gum disease and may need to have their teeth cleaned every three months.”</p>

<p><strong>5. Noted. You haven’t mentioned gingivitis, but it’s the disease we all hear about in oral-care advertisements. What exactly is it?</strong></p>

<p>“Gingivitis is the precursor to periodontitis. Red, inflamed, and bloody gums are indicators, but the good news is that gingivitis is reversible. When gingivitis is left untreated, it progresses to the more aggressive periodontitis, which has irreversible effects on gum tissue, ligaments, and bone.”</p>

<p><strong>6. Are any foods particularly bad for the gums? Or any that are good?</strong></p>

<p>“There aren't necessarily good or bad foods for the gums. We all know different foods that affect our enamel health: sugars from soda, candy, and carbohydrates, acid from coffee and citrus, and sticky or hard foods. When it comes to healthy teeth and gums, it all boils down to two things: proper, thorough oral hygiene and regular visits to the dentist.”</p>

Brushing your teeth regularly is part of good oral hygiene, but if you brush too vigorously, you can cause more harm than good. Brushing your teeth too hard can wear down enamel, irritate your gums, make your teeth sensitive to cold, and even cause cavities. To avoid these problems, Price recommends using a soft bristled toothbrush. "Just look for the letters ADA [American Dental Association] on the box, which means the bristles are firm enough to remove plaque, but soft enough not to cause damage," he advises.</span></p>

For some people, stress can trigger frequent clenching of the jaw or grinding of the teeth .
